Responsibilities:
- Conduct in-home assessments and health screenings.
- Provide patient education based on care needs.
- Identify diagnoses for care management.
- Collaborate with medical team to create care plans.
- Coordinate care with PCPs, specialists, and providers.
- Lead clinical team including nurses and coordinators.
Requirements:
- Master’s degree from accredited NP/PA program.
- 1+ year of clinical or home care experience.
- EMR experience preferred.
- Experience in geriatric care preferred.
- Active Nevada NP/PA license and certifications.
- Valid Nevada driver license and automobile insurance.
- Proficient in Microsoft Office and MS Teams.
