Responsibilities:

• Provide compassionate, individualized, and holistic patient care.
• Deliver clinical care, guidance, and education virtually.
• Perform emergent and urgent telephonic triage for patients.
• Coordinate care for acute or limited duration patient needs.
• Develop, implement, and modify individualized action plans.
• Supervise LPN and/or MA activities as requested.
• Document patient care activities accurately and timely.
• Integrate clinical practice, data dashboards, and processes.
• Promote and participate in patient engagement initiatives.
• Participate in care team, operational, and clinical meetings.
• Adhere to medical policies, protocols, and clinical guidelines.
• Follow compliance policies, procedures, and standards.

Requirements:

• Bachelor’s degree in Nursing or commensurate clinical experience.
• Active nurse licensure in Rhode Island or multistate license.
• Recent telephonic triage, urgent care, or ER/ED experience.
• Clinical experience caring for aging adults and complex needs.
• Advanced training in geriatrics and/or palliative medicine preferred.
• Previous experience with managed care or value-based delivery preferred.
• Average to advanced computer and software skills required.
