Responsibilities
- Conduct monthly phone calls with patients, practices, pharmacies
- Spend 7-8 hours on the phone daily
- Create/revise personalized care plans with practices/providers
- Identify social determinants of health, care gaps, eligibility
- Coordinate with office staff and community resources
- Empower patients to manage their wellness goals
- Make 30-50 calls daily to complete 20 minutes of CC service
- Maintain patient privacy, confidentiality, and safety
Requirements
- Exceptional verbal and writing skills in English and Spanish
- At least 1 year experience as an MA with active accreditation
- Experience with EMR/EHR systems, Microsoft Office, GoogleSuite
- Call Center and/or Triage experience is a plus
- Experience with a geriatric population is a plus
- Case management or care coordination experience is a plus
