Summary:
- District Health Care Services is looking for a Community Support Worker (CSW) to assist consumers in achieving rehabilitation and recovery goals as defined in the Treatment Plan.
- The CSW works primarily in the community or in the consumer’s natural setting.
- This position is an Independent Contractor role that involves patient support activities in accordance with District Health Care Services and Department of Behavioral Health requirements.
- 90% of the work is community-based or occurs in the consumer’s natural environment.
Responsibilities:
- Manage a caseload of no more than 40 adults.
- Assist the consumer in developing interpersonal and community coping skills.
- Help the consumer develop relapse and symptom management strategies and plans.
- Provide and coordinate referrals to ensure consumers receive the requested assistance.
- Conduct visits to the consumer’s job site, home, hospital room, and other natural settings.
- Transport consumers in a personal vehicle as needed or coordinate transportation.
- Adhere to District documentation requirements.
- Submit billing/progress notes as required.
- Provide crisis intervention to consumers as necessary.
- Meet assigned weekly/monthly productivity requirements (25 billable hours per week).
- Perform other duties as assigned.
Requirements:
- Associates Degree or three years of experience in a relevant human service field.
- One year of experience in human service activities.
- Strong customer service background.
- Knowledge of medical terminology and insurances.
- Good oral, written, and telephone communication skills.
- Ability to work independently or in a team-oriented environment and interrelate well with individuals from diverse ethnic and cultural backgrounds.
- Basic working knowledge of computers.
