Responsibilities:

• Provide high-quality virtual individual counseling to members.
• Create detailed care plans with goal setting and outcome tracking using aptihealth tools.
• Collaborate closely with care teams.
• Track and document patient progress and clinical findings.

Requirements:

• Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or equivalent license.
• Unrestricted New York license required.
• Minimum 3 years of experience in individual counseling.
• Competency in providing counseling to diverse populations using varied approaches.
