Substance Use Disorder (SUD) Clinical Care Navigator at Spring Health Full Time

Responsibilities

• Telephonic screening, brief intervention, and referral (SBIRT model)
• Apply active listening, psycho-education, and motivational interviewing
• Utilize ASAM criteria, facilitate referral, and coordinate care
• Provide consultation on mental health concerns and family issues
• Provide support and safety planning for crisis situations
• Utilize written communication skills via email
• Document information, maintain records, and track progress
• Identify and solve barriers or disruptions to care
• Interact with multi-disciplinary teams for smooth member experience
• Participate in staff meetings, case consultations, and trainings
• Assist with onboarding and mentoring new team members
• Follow Spring Health policies and maintain confidentiality

Requirements

• Required mental health license: LCSW, LMFT, LPC
• Minimum of 2+ years of SUD clinical care experience
• Experience with motivational Interviewing, SBIRT, and Triage
• Knowledge of ASAM criteria, contingency management, and MAT
• Excellent listening, written, and verbal communication skills

• Tech-savvy with ability to use various computer platforms
• Able to multi-task and context switch
