Responsibilities
- Accountable for Part B performance including PMPM targets and referrals
- Drive value-based care outcomes around referral patterns and programs
- Develop clinical strategies for dementia care and geriatrics initiatives
- Collaborate on design and implementation of care programs
- Review utilization data to inform specialty-specific strategy
- Provide virtual consultation on complex patients
- Direct care for patients approximately one day a week
- Travel to meet stakeholders and partners (10-20% travel)
Requirements
- Must be a licensed physician within commuting distance of a clinic
- Board-Certified in Geriatrics, Internal Medicine, or Family Medicine
- Minimum 2 years designing integrated specialty care programs
- Minimum 7 years in direct clinical value-based care
- Experience with team-based care and excellent collaboration skills
- Deep understanding of value-based payment models and program design
- Comfortable with analytics and presenting business cases
