Responsibilities
- Live our mission of “restoring humanity to healthcare”
- Manage patients with complex conditions via tele-assist home visits
- Complete thorough initial and ongoing follow-up visits with patients
- Conduct post-discharge visits, including medication reconciliation
- Engage with patients who haven’t seen their primary care provider
- Collaborate with the patient’s high-risk care team and specialists
- Respond to patient and family telephone calls with team support
- Participate in the market after-hours call pool
Requirements
- Completed an accredited NP program with national certification
- 5+ years of Nurse Practitioner experience with complex care
- Bilingual in Spanish and English highly preferred
- Currently licensed in Washington and willing to obtain Arizona license
- Based in PST time zone and flexible with PT/MT hours
- Experience with home-based care in the Seattle area preferred
- Strong technology skills and ability to use telehealth platforms
